Total Life, Inc. in Phoenix invites qualified nurse practitioners for an in-person contract role to lead longevity days at partner senior living communities in the West Valley.

Each day begins with a brief educational talk on healthy aging, using materials we provide, followed by one-on-one longevity visits with residents who sign up. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's content.

Qualificações

• Active, unrestricted Arizona APRN license with prescribing authority.
• Current national NP board certification.
• Experience caring for adults 65+.
• Experience with Medicare documentation and coding.

Responsabilidades

• Conduct longevity visits and comprehensive geriatric assessments.
• Evaluate residents for weight management and GLP-1 therapy when appropriate.
• Coordinate follow-up care with primary care providers and specialists.
• Document encounters to Medicare standards in our EHR.
• Travel to assigned senior living communities in the West Valley.
• Represent Total Life professionally with residents, families, and staff.
