Responsibilities

• Support the Admissions team with incoming referrals and potential admissions.
• Gather, review, and organize referral and admission documentation.
• Help ensure required information is received prior to admission.
• Communicate with hospitals, case managers, referral sources, patients, and families.
• Assist with insurance, demographic, and admission information as needed.
• Prepare admission paperwork and admission packets.
• Help patients and families understand the admission process and what to expect.
• Communicate upcoming admissions to Nursing, Therapy, Dietary, Business Office, and other departments.
• Track referrals and admission status.
• Maintain accurate records and documentation.
• Assist with follow-up calls and referral communication.
• Provide excellent customer service to patients, families, visitors, and referral partners.
• Support the Admissions and Marketing team with administrative and coordination needs.
• Assist with facility tours and inquiries as needed.
• Help ensure patients and families receive a warm, organized welcome upon arrival.
