Job summary

Vintage Wellness & Rehabilitation is seeking an Experienced Admission Professional to manage the admissions process, marketing programs, and information gathering for prospective residents. The role focuses on screening applicants, guiding them through the transition, and supporting census development while keeping families informed.

The Admissions Coordinator will maintain admission guidelines, verify financial eligibility, schedule tours, and collaborate with referral sources to drive quality

Qualifications

• High school diploma required.
• Experience in health care services desirable.
• Familiarity with long-term care and/or health care services useful.
• Presents professional image through appearance, behavior and speech.

Responsibilities

• Maintains admissions guidelines and actively participates in writing, updating, and recommending changes to admissions criteria, policies and procedures.
• Markets programs and facilities by providing information, answering questions, conducting tours with prospective residents and referral sources.
• Handles walk-in or telephone inquiries and invites referral sources to nursing center; completes inquiry system for admissions, Medicare facts, and room rates; gathers nursing information from the e-discharge system or floor nurse at the hospital.
• Ensures financial verification is accurate and complete before the resident is admitted to the Facility thus assisting in reducing accounts receivable.
• Screens prospective residents by comparing resident's condition to admission criteria; evaluating and accepting or rejecting residents; referring residents and family to other programs and institutions.
• Resolves resident/family dissatisfactions by investigating concerns; recommending changes in service policies and procedures.
• Maintains necessary knowledge of served market areas, resident origin, and major referral sources.
• Assists with census development efforts by making quality contact with referral sources.
• Manages incoming inquiries, as needed, dependent on sales model (i.e. central intake)
• Uses clinical inventory to evaluate residents for admission; when necessary, visits prospective residents in the hospital and makes assessment regarding suitability; Contacts Social Services to verify eligibility; Contacts insurance companies to determine coverage; Meets with families upon or before admission to sign paperwork; Records information identifying physician/resident, room type, insurance, and admission date; Inspects resident’s room prior to admission; participates in morning kick-off meeting.
• Effectively sells private transition days to current residents.
• May assist Administrator in contacting families after admission and discharge to determine satisfaction; follow-up on complaints with residents and staff.
• Other duties may change or be assigned at any time.
