Exalt Health, LLC. is seeking an Admissions Coordinator to facilitate patient admissions to our inpatient rehabilitation hospital. You will coordinate with patients, families, referral sources, and the hospital team to ensure a seamless transition into care.

The role involves handling patient intake, verifying insurance, obtaining pre-authorizations, and maintaining accurate EMR records. Strong communication and organization are essential in a fast-paced healthcare setting.

Qualifications

• High school diploma or equivalent required; Associate's degree preferred.
• 1-2 years of healthcare admissions, patient registration, or case management experience.
• Knowledge of medical terminology and insurance verification processes.

Responsabilités

• Field patient and referral inquiries and initiate the admission process.
• Verify insurance coverage and obtain pre-authorizations for inpatient rehabilitation.
• Coordinate with physicians, therapists, and case managers to gather medical documentation.
• Schedule admissions and coordinate transportation as needed.
• Maintain patient records in the EMR/admissions system and update demographic/insurance details.
• Provide exceptional customer service to patients and families throughout the admission process.
