Job summary

CVS Health in New Jersey is seeking a Case Manager Registered Nurse - Field to deliver quality healthcare services across Hudson County and surrounding counties. This role combines field visits with remote work, requiring strong assessment, care planning, and collaboration with PCPs and care teams.

The position offers a Monday–Friday schedule, up to 75% travel, and a focus on utilization management and program policies.

Qualifications

• Must reside in Hudson & Surrounding Counties in New Jersey.
• 3+ years clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care.
• Active and Unrestricted RN license in NJ.
• Must possess reliable transportation and be willing and able to travel up to 75% of the time in Hudson and surrounding counties In New Jersey. Mileage is reimbursed per our company expense reimbursement policy.
• Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.

Responsibilities

• Conduct home visits using assessment tools for MLTSS/D-SNP members.
• Manage pediatric and medically complex cases when assigned.
• Schedule and attend interdisciplinary meetings; advocate for safe discharges.
• Collaborate with member and care team to develop cost-effective care plans.
• Document accurately in the member's electronic health record.
• Coordinate care with member, PCP, and other team members.
• Mentor new hires after achieving proficiency.
