Job summary

CVS Health is seeking a Case Manager Registered Nurse - Field to deliver comprehensive care management across Bergen and surrounding counties in New Jersey. The role involves timely care planning, collaboration with providers, and ensuring appropriate utilization of services in managed programs.

Responsibilities include conducting in-home assessments, coordinating with interdisciplinary teams, and documenting in the EHR while mentoring newer staff.

Qualifications

• 3+ years clinical practice experience (hospital, home health, or ambulatory care).
• Active and Unrestricted NJ RN license.
• Willingness to travel up to 75% in Bergen & surrounding counties, NJ.
• NJ Choice certification required (80%+).

Responsibilities

• Conduct face-to-face home Visits for MLTSS/DSNP/FIDE members.
• Attend interdisciplinary meetings and advocate for safe discharge.
• Develop care plans with the care team and authorize services cost-effectively.
• Document findings timely in electronic health record.
• Coordinate care with member, PCP, and care team.
• Mentor new hires after achieving proficiency.
