Job summary

CVS Health is seeking a Case Manager Registered Nurse - Field to develop and implement health service strategies for members, with up to 75% field travel in Hudson County and surrounding counties. Typical schedule is Monday through Friday, 8 a.m. to 5 p.m. EST.

You will conduct home visits, create care plans, coordinate with PCPs and care teams, document in the EHR, and mentor new hires as you grow in the role.

Qualifications

• Must reside in Hudson and/or surrounding counties in New Jersey.
• 3+ years clinical practice experience (hospital or home health/ambulatory care).
• Active and Unrestricted RN license in NJ.
• Willing and able to travel up to 75% in Hudson and surrounding counties; mileage reimbursed.

Responsibilities

• Conduct face-to-face visits in members' homes using assessment tools for MLTSS/DSNP/FIDE and non-MLTSS members.
• Coordinate care with member, care team, PCP, and other participants.
• Develop care plans and authorize services within MLTSS/FIDE benefits.
• Document accurately in the member's electronic health record.
• Lead interdisciplinary meetings and mentor new hires.
