CVS Health is seeking a Registered Nurse Case Manager – Field to coordinate care for members with complex needs in Morris County and surrounding areas. This role combines field visits with occasional remote work, promoting safety and independence at home.

You will conduct in-home assessments, develop care plans, and collaborate with interdisciplinary teams. A NJ RN license and 3+ years clinical experience are required, with travel up to 75%.

Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum 3 years clinical experience in hospital, home health, or ambulatory care.
• Residence in Morris County or nearby communities.
• Frequent local travel and in-home visits.

Responsibilities

• Conduct in-home assessments for MLTSS program members.
• Evaluate medical, behavioral, and social needs to guide care.
• Collaborate with providers, caregivers and teams for holistic care.
• Develop and manage individualized care plans for safety and independence.
• Participate in care team meetings and advocate for members.
