Job summary

CVS Health Corporation is seeking a Registered Nurse Case Manager - Field to coordinate care for members with complex needs in home and community settings. This is primarily field-based, with the flexibility to work from home when not in the field, offering independence and purpose-driven work.

You will perform in-home assessments, develop and manage care plans, and collaborate with providers while travel up to 75% across Morris County and surrounding areas.

Qualifications

• Active, unrestricted RN license in New Jersey (NJ)
• At least 3 years of clinical experience (hospital, home health, ambulatory care, or similar)
• Residence in Morris County or nearby communities
• Comfort with frequent local travel and in-home visits
• Strong assessment, communication, and critical-thinking skills
• NJ Choice Certification completion with minimum exam score 80% and state training modules
• Ability to work independently with a flexible daily schedule

Responsibilities

• Conduct in-home assessments for members enrolled in MLTSS programs
• Evaluate medical, behavioral, and social needs to support care
• Collaborate with providers, caregivers, and interdisciplinary teams
• Develop and manage individualized care plans promoting safety and quality of life
• Participate in care team meetings and advocate for member needs
• Document assessments and activities in the EHR
• Support care transitions and link to community resources
• Mentor new team members as you grow in the role
