CVS Health seeks a Registered Nurse Case Manager – Field to coordinate care for members with complex needs across home visits in New Jersey. This role emphasizes independence, community engagement, and evidence-based planning with a field-based schedule and travel up to 75%.

The position requires an active NJ RN license, 3+ years of clinical experience, and successful completion of the NJ Choice Certification (80% minimum) including state training modules and mentoring components.

Qualifications

• Active, unrestricted RN license in New Jersey required.
• Minimum 3 years of clinical experience in hospital, home health, ambulatory care, or similar settings.
• Successful completion of the NJ Choice Certification with minimum exam score of 80% and completion of state training modules.

Responsibilities

• Conduct in-home assessments for MLTSS program members.
• Evaluate medical, behavioral, and social needs to support appropriate care.
• Collaborate with providers, caregivers, and interdisciplinary teams to ensure coordinated care.
• Develop and manage individualized care plans to promote safety and independence.
• Document assessments and care activities in the electronic health record.
• Support care transitions and connect members with community resources.
• Mentor new team members as you grow in the role.
