Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum of 3 years of clinical experience in hospitals, home health, or ambulatory care.
• Residency in Middlesex County, NJ.
• Must possess reliable transportation and be willing to travel up to 75% of the time.
• Completion of NJ Choice Certification with an 80%+ exam, state training modules, and field mentoring.

Responsibilities

• Conduct face-to-face home visits and assessments for MLTSS/D‑SNP/FIDE populations.
• Coordinate with providers and care teams to develop individualized care plans.
• Document assessments, care planning, and interactions in the EHR.
• Mentor new staff after achieving proficiency.
• Complete NJ Choice Certification requirements.
