Job summary

CVS Health is seeking a Case Manager Registered Nurse - Field to support members in Union County and surrounding areas. This role blends field visits with remote work, requiring substantial travel to coordinate complex care plans and connect members with needed services.

You will lead assessments, develop care plans, collaborate with interdisciplinary teams, and advocate for cost-effective, high-quality care while mentoring newer staff.

Qualifications

Must reside in Union County, NJ.
3+ years of clinical practice in hospital or home health/ambulatory care.
Active and unrestricted NJ RN license.
Willing to travel 50–75% within Hudson and surrounding counties in NJ.
Successful completion of NJ Choice certification required.

Responsibilities

Perform face-to-face home visits to assess medical needs under MLTSS/D-SNP programs.
Develop care plans and authorize cost-effective services with care team.
Document accurately in the member's electronic health record.
Coordinate with member, PCP, and care team; advocate for safe discharge.
Mentor new hires after attaining proficiency.
