Job summary

CVS Health is seeking a Case Manager Registered Nurse - Field to support members across Union and surrounding counties, NJ. This role combines field visits with remote work, requiring strong clinical judgment and care coordination.

You will conduct home visits, develop care plans, coordinate with PCPs, authorize services, document in the EHR, and mentor new hires. Travel up to 75% and schedule Mon-Fri 8am-5pm EST.

Qualifications

• 3+ years clinical practice experience (hospital/ambulatory/home health)
• Active and Unrestricted RN license in NJ
• Must reside in Union County, NJ and travel 75% in NJ

Responsibilities

• Conduct face-to-face home visits for MLTSS and D-SNP/FIDE members to assess medical needs.
• Possibly pediatric or medically complex cases.
• Schedule and attend interdisciplinary meetings; advocate for proper discharge and services.
• Develop care plans and authorize services within MLTSS/FIDE, cost-effectively.
• Document accurately in the member's EHR.
• Coordinate with member/representative, PCP, and care team.
• Mentor new hires after proficiency is reached.
