CVS Health Corporation is seeking a compassionate Registered Nurse Case Manager for field-based work in Morris County, NJ. This role blends independent in-home assessments with collaborative care planning to support members with complex needs in living safely at home.

Ideal candidates hold an active NJ RN license, have 3+ years of clinical experience, and can travel locally. A hybrid schedule allows work from home when not in the field, with strong emphasis on documentation and interdisciplinary

Qualifications

• Active, unrestricted RN license in New Jersey.
• At least 3 years of clinical experience in hospital, home health, ambulatory care, or similar settings.
• Residence in Morris County or nearby communities.
• Comfort with frequent local travel and in-home visits.
• Strong assessment, communication, and critical-thinking skills.
• NJ Choice Certification completed with Minimum exam score of 80% and state training modules.
• Ability to work independently and manage a flexible daily schedule.

Responsibilities

• Conduct in-home assessments for members in MLTSS programs.
• Evaluate medical, behavioral, and social needs to guide care and services.
• Collaborate with providers, caregivers, and interdisciplinary teams for coordinated care.
• Develop and manage individualized care plans promoting safety and independence.
• Participate in care team meetings and advocate for member needs.
• Document all assessments and care activities in the EHR.
• Support members through care transitions and connect with community resources.
• Mentor new team members as you grow in the role.
