Job summary

CVS Health is seeking a compassionate Registered Nurse Case Manager to join our field team and help coordinate care for members with complex needs in Morris County and nearby areas.

This in-home, field-based role offers flexibility to work from home when not in the field, 40 hours per week, and the opportunity to partner with providers and Care Teams to improve safety, independence and quality of life for patients.

Qualifications

• Active, unrestricted RN license in New Jersey.
• 3+ years of clinical experience (hospital, home health, ambulatory care, or similar).
• Residence in Morris County or nearby communities.
• NJ Choice Certification completed with minimum 80% exam score.
• Ability to travel frequently and perform in-home visits.
• Strong assessment, communication, and critical-thinking skills.

Responsibilities

• Conduct in-home assessments for members enrolled in MLTSS programs.
• Evaluate medical, behavioral, and social needs to support appropriate care.
• Collaborate with providers, caregivers, and interdisciplinary teams for coordinated care.
• Develop and manage individualized care plans to promote safety and independence.
• Participate in care team meetings and advocate for member needs.
• Document assessments and care activities in the EHR.
• Support members through transitions and connect with community resources.
• Mentor new team members as you grow in the role.
