Job summary

ForzaCare, part of Ethos Risk Services, is hiring a Field Medical Case Manager to coordinate care for injured workers and lead return-to-work plans. You will partner with medical providers, employers, and insurers to deliver timely, transparent care management.

Ideal candidates are licensed RNs or CRCs with relevant workers’ compensation experience; CCM/CIRS credentials are a plus. A valid driver’s license and reliable transportation are required.

Qualifications

• Active RN or Certified Rehabilitation Counselor (CRC) license with related degree is required.
• CCM, CIRS or similar case management credentials are preferred.
• Must comply with state licensure and certification requirements and have a valid driver’s license with reliable transportation.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits at employer sites.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.

Education
