Job summary

ForzaCare, part of Ethos Risk Services, seeks a Field Medical Case Manager to guide injured workers through recovery and return-to-work processes. You will coordinate among medical providers, employers, and insurance carriers to ensure timely, transparent care management and documentation of progress with stakeholders.

The ideal candidate is an active RN or CRC with related licensure; CCM/CIRS certifications are a plus.

Qualifications

• Active RN license or CRC credential required.
• CCM, CIRS or related case-management certifications preferred.
• Must meet state licensure requirements and be able to travel to appointments.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, possibly at employer sites or workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers and internal teams.
