Job summary

ForzaCare is hiring a Field Medical Case Manager to coordinate care for injured workers and return-to-work plans. The role requires an active RN or CRC license with a medical background in workers' compensation and travel to appointments as needed.

Strong organizational, collaborative, and communication skills are essential. Prior workers' compensation case management experience is highly valued, and CCM/CIRS credentials are a plus.

Qualifications

• Active RN or CRC license with degree is required.
• CCM, CIRS or other case-management credentials are preferred.
• State licensure requirements must be met and travel is required.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to workplaces as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide regular progress reports to clients and stakeholders.
• Ensure case management meets customer and regulatory standards.
• Build and maintain relationships with clients, providers and team members.
