Job summary

ForzaCare seeks a Field Medical Case Manager to coordinate care for injured workers, connecting providers, employers and insurers to enable timely recovery and return-to-work. You’ll attend appointments and visits at worksites as needed, developing individualized recovery goals and documenting progress for stakeholders.

Ideal candidates are licensed RNs or CRCs, with CCM/CIRS or similar credentials preferred, and willingness to travel.

Qualifications

• Active RN license or CRC certification required.
• Travel to appointments with insured patients is expected.
• Workers' compensation case management experience preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments including visits to employers and workers' worksites.
• Develop, document, and monitor recovery goals and return-to-work plans.
• Provide regular progress reports to clients and stakeholders.
• Ensure compliance with customer and regulatory requirements.
• Build strong relationships with clients, providers and team members.
