Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return-to-work. You will coordinate care among medical providers, employers, and insurers, attending appointments as needed and developing personalized recovery plans.

Ideal candidates are licensed RNs or CRCs; prior workers' compensation experience is strongly preferred. Travel to appointments is required, with a focus on timely, compliant, and compassionate care management.

Qualifications

• Active RN or CRC licensure is required.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure requirements.
• Prior workers' compensation case management experience preferred.
• Has a valid driver's license and travel ability.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at employers' or workers' locations as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal team members.
