Job summary

ForzaCare, part of Ethos Risk Services, seeks a Field Medical Case Manager to coordinate care for injured workers and guide their recovery back to work. The role connects workers, medical providers, employers, and insurers to ensure timely, transparent care and compliant management.

Licensed nurses or certified rehabilitation counselors are ideal candidates. Prior workers' compensation experience is strongly preferred, and travel to appointments is required as needed.

Qualifications

• Active RN or CRC license with an associated college degree is required.
• CCM, CIRS or other case management credentials are preferred.
• Must comply with state licensure and certification requirements.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with the injured workers, potentially at their workplace.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
