Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate care for injured workers and guide them through recovery and return-to-work plans. You will connect the injured worker, medical providers, employers, and insurance carriers to ensure timely, transparent care management.

Licensed RN or CRC is required; CCM/CIRS or other credentials are a plus. Prior workers' compensation experience is highly valued. Travel to appointments and visits to worksites may be required.

Qualifications

• Active RN or CRC license with a related college degree is required.
• CCM, CIRS, or other case management credentials are preferred.
• Prior workers' compensation case management experience is strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers and may visit their place of employment.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
