Job summary

ForzaCare seeks a Field Medical Case Manager to coordinate recovery and return-to-work plans for injured workers. You will serve as the central point linking workers, medical providers, employers, and insurers to ensure timely, transparent, and effective care management.

Ideal candidates hold an active RN or CRC license; CCM/CIRS credentials are a plus. Prior workers' compensation case management experience is strongly preferred, with willingness to travel and maintain compliance across cases.

Qualifications

• Active RN or CRC license required.
• CCM, CIRS or other case management credentials preferred.
• Travel to appointments with valid driver's license and reliable transportation.

Responsibilities

• Coordinate care between medical providers, employers, and insurers.
• Attend appointments with injured workers, including on-site visits at workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
