Job summary

ForzaCare seeks a Field Medical Case Manager to coordinate recovery and return-to-work efforts for injured workers. You will connect patients with providers, employers, and insurers to ensure timely, transparent care management.

Ideal candidates are licensed RNs or CRCs, with CCM/CIRS credentials preferred. Travel to appointments is required, and prior workers' compensation experience is highly valued.

Qualifications

• Active RN or CRC licensure is required.
• CCM, CIRS or other case management credentials are preferred.
• Must comply with state-specific licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.
• Valid driver's license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers and injured workers.
• Attend appointments with injured workers, including on-site visits to workplaces or locations.
• Develop, document and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers and internal team members.
