Job summary

ForzaCare, part of Ethos Risk Services, is seeking a Field Medical Case Manager to guide injured workers through recovery and return-to-work. You will coordinate among medical providers, employers, and insurers to ensure timely, transparent care management.

Ideal candidates are licensed RNs or CRCs; CCM/CIRS credentials are a plus. Travel to appointments is required, with a valid driver's license and reliable transportation essential.

Qualifications

• Active RN or Certified Rehabilitation Counselor license with degree is required.
• CCM, CIRS or similar case management credentials are preferred.
• Must meet state licensure and certification requirements.
• Experience in workers' compensation case management is strongly preferred.
• Valid driver's license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including at their place of employment.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
