Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate recovery and return-to-work for injured workers. You will serve as the central point connecting workers, medical providers, employers, and insurers to ensure timely, transparent care management.

Ideal candidates are licensed nurses or CRCs. Prior workers' compensation experience is preferred, and willingness to travel to appointments is required. This is a field-based, full-time role with a focus on compliant, high-quality care.

Qualifications

• Active RN or CRC license is required.
• CCM, CIRS or other case management credentials are preferred.
• Must meet state licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.
• Valid driver's license and ability to travel to appointments.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including site visits at workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
