Job summary

ForzaCare seeks a Field Medical Case Manager to coordinate care for injured workers and support safe return-to-work plans.

You will connect providers, employers, and insurers to ensure timely, transparent care and travel to patient sites as needed.

Active RN or CRC license is required; CCM/CIRS credentials are a plus. Prior workers' compensation experience is strongly preferred.

Qualifications

• Active RN or CRC license required.
• CCM, CIRS or other case management credentials preferred.
• Must travel to appointments and maintain reliable transportation.

Responsibilities

• Coordinate care between medical providers, employers, and insurers.
• Attend appointments with injured workers and may visit worksites.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide progress reports to clients and stakeholders.
• Ensure case management meets customer and compliance requirements.
• Build strong relationships with clients, providers, and team members.
