Qualifications

• Must hold BA/BS and 2+ years in a social work setting.
• Experience with LTSS/IDD and behavioral health a plus.
• Strong collaboration with care team and families.

Responsibilities

• Perform face-to-face program assessments using predefined questions and motivational interviewing techniques.
• Identify members with potential clinical health care needs and coordinate cases with the interdisciplinary team.
• Manage non-clinical needs of members to ensure cost-effective utilization of LTSS/IDD and related services.
• Document short- and long-term service goals with the member and care team.
• May mentor or train new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state/county agencies as required.
• Assist with appeals, grievances, and state audits.
