Elevance Health in Ohio is seeking a LTSS Service Coordinator (Case Manager) to manage a designated caseload and lead the Person Centered Planning process. You will document preferences, conduct assessments, and develop comprehensive care plans in collaboration with medical directors and an interdisciplinary team.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Preferred: case management experience with older adults or individuals with disabilities.
• BA/BS in Health/Nursing preferred.

Responsabilités

• Perform field-based assessments and coordinate care using motivational interviewing.
• Serve as single point of contact coordinating LTSS/IDD, BH or PH needs with clinical teams.
• Develop and manage Person Centered Plans and backup plans for designated caseload.
• Document goals with care teams and identify candidates for alternative service levels or waiver programs.
• Mentor or train new staff and participate in process improvements.
• Submit utilization requests with supporting care plans and report critical incidents to authorities.
