Job summary

Elevance Health is seeking a LTSS Service Coordinator in Ohio to manage designated caseloads, conduct assessments, and develop Person-Centered Support Plans. You will collaborate with medical directors and an interdisciplinary team to deliver integrated care aligned with state and federal requirements.

The role involves field-based work across Ohio, with occasional office meetings, mentoring responsibilities, and the potential for process improvement contributions.

Qualifications

• Requires BA/BS degree and at least 2 years of social work experience.
• Experience with waiver programs (LTSS/IDD) and care coordination preferred.
• Strong communication and documentation skills for PCSP and care team.

Responsibilities

• Perform face-to-face program assessments using predefined questions and motivational interviewing.
• Identify members with potential clinical needs and coordinate cases with the clinical team.
• Manage non-clinical needs of members with chronic illnesses to optimize LTSS utilization.
• Document member goals in collaboration with their care team and caregivers."
• Mentor or preceptor for new staff and participate in process improvements.
