Job summary

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) to manage a designated caseload in Ohio. The role involves person-centered planning, documenting preferences, conducting assessments, and developing comprehensive PCSPs with backup plans.

You will collaborate with medical directors and the care team to create an integrated care plan while engaging the member’s support network. The position focuses on LTSS/IDD, BH or PH needs, and requires strong case management and motivational

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Preferred: case management experience with older adults or individuals with disabilities.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Perform face-to-face program assessments using pre-defined questions to identify needs and apply motivational interviewing techniques.
• Coordinate and manage waivers (LTSS/IDD, BH or PH needs) and act as the single point of contact with the interdisciplinary team.
• Manage non-clinical needs of members with chronic illnesses to ensure efficient use of long-term services and supports.
• Document member goals in collaboration with their care team and identify candidates for alternative services or waiver programs.
• Mentor or precept new staff and contribute to process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to appropriate agencies and participate in appeals, grievances, and state audits.
