Job summary

Elevance Health is seeking an LTSS Service Coordinator (Case Manager) to support a designated Ohio caseload. This field-based role travels to client sites and partners with interdisciplinary teams to deliver person-centered planning and comprehensive care coordination.

You will conduct assessments, document PCSPs, manage waiver needs, mentor staff, and ensure compliance with state and federal regulations while engaging trusted networks and caregivers. Columbus, OH area preferred.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience with a social work agency.
• Experience with LTSS, IDD, and BH/PH needs and waiver programs preferred.
• Ability to coordinate with a care team and document PCSPs.

Responsibilities

• Perform face-to-face program assessments and identify care needs using predefined tools.
• Coordinate member cases with clinical management and multidisciplinary teams.
• Manage non-clinical needs for members with chronic illnesses or disabilities.
• Document short and long-term goals with the member and care team.
• Mentor or precept new staff and contribute to process improvements.
• Submit utilization/authorization requests with supporting care plan docs.
• Report critical incidents to appropriate state/count y agencies.
• Assist with appeals, grievances, and state audits.
