Elevance Health seeks a LTSS Service Coordinator to deliver person-centered care coordination across Ohio. The role involves assessing, planning, and documenting plans with the member and care team, while working with Medical Directors and an inter-disciplinary group to ensure integrated care.

The position requires a BA/BS with 2 years in social work or related work, and a preference for experience with older adults or disabilities. Travel and hybrid/onsite work are expected.

Qualifications

• Requires BA/BS degree and minimum of 2 years of experience working with a social work agency.
• Preference for case management experience with older adults or individuals with disabilities.
• BA/BS in Health/Nursing preferred.

Responsabilités

• Perform face-to-face program assessments using predefined questions and motivational interviewing techniques.
• Coordinate cases with clinical teams to address LTSS/IDD, BH or PH needs.
• Manage non-clinical needs of members with chronic illnesses and disabilities to ensure efficient utilization of long-term services.
• Document member preferences, goals, and care plans with their care team and caregivers.
• Submit utilization/authorization requests and support state audits and appeals as needed.
