Job summary

Elevance Health in Ohio is seeking an LTSS Service Coordinator (Case Manager) to manage a designated caseload across statewide programs. The role involves field-based client site visits with occasional office attendance, and collaboration with medical directors for integrated care planning.

The position requires a BA/BS and at least 2 years of social work experience, with a preference for case management in older adults or disabilities.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.

Responsibilities

• Perform face-to-face program assessments for waiver needs using pre-defined questions and motivational interviewing techniques.
• Coordinate cases with the clinical healthcare management and interdisciplinary team to provide care coordination support.
• Manage non-clinical needs of members with chronic illnesses or disabilities to ensure efficient utilization of long-term services and supports.
• Document short and long-term service goals in PCSP with the member and care team; identify alternative levels of service as needed.
• Mentor or preceptor for new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with documentation supporting the care plan.
• Report critical incidents to appropriate state or county agencies.
• Assist with appeals, grievances, and state audits.
