Elevance Health is seeking a LTSS Service Coordinator to conduct service coordination for a defined caseload in specialized programs. The role involves person-centered planning, assessment, and development of comprehensive care plans, engaging the member’s support circle, and coordinating LTSS/BH/PH needs.

The position emphasizes field-based activities with travel to client sites, collaboration with medical directors, and participation in interdisciplinary care rounds to support integrated care.

Qualifikationen

• BA/BS degree.
• Minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Aufgaben

• Perform face-to-face program assessments for identification and coordination of LTSS/IDD and BH or PH needs.
• Coordinate member cases with clinical healthcare management and interdisciplinary team as the single point of contact.
• Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective LTSS utilization.
• Document member short- and long-term service goals with the care team and caregivers.
• Identify members who could benefit from alternative services or other waiver programs.
• Mentor or precept with onboarding and participate in process improvements.
• Submit utilization/authorization requests with documentation aligned to the care plan.
• Report critical incidents to state and county agencies as required.
• Assist with appeals, grievances, and state audits.
