Job summary

Elevance Health is seeking a LTSS Service Coordinator to manage a defined caseload in field settings, collaborating with the member and their care team to develop and implement Person Centered Plans and backup strategies. You will engage directly with clients in community locations, adjusting plans per state requirements.

The role requires a BA/BS and at least two years in social work, with travel to client sites and occasional office meetings.

Qualifications

• BA/BS degree required.
• Minimum of 2 years of experience in a social work setting or equivalent.
• Education/experience may vary by state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments for LTSS/IDD and BH/PH needs.
• Coordinate cases with clinical teams to provide care coordination.
• Manage non-clinical needs of members with chronic illnesses to ensure efficient LTSS use.
• Document member goals with the care team and supporters.
• Identify members who may need alternative service levels or other waivers.
• Mentor or train new staff and participate in process improvements.
• Submit utilization/authorization requests to utilization management.
