Job summary

Elevance Health in Ohio seeks an LTSS Service Coordinator – RN Clinician (Case Manager) to manage member care plans across physical health, behavioral health, long‑term services, and psychosocial needs. This field-based role involves travel to client sites with occasional office attendance.

The RN must reside in Ohio and hold an active license. A high school diploma is required with at least 3 years in case management or similar work. A BA/BS in Health/Nursing is preferred.

Qualifications

• Performs telephonic and in-person assessments to identify, evaluate, coordinate and manage member needs.
• Develops individual care plans in collaboration with members, families and clinicians.
• Works with the care team to optimize member health across the care continuum.

Responsibilities

• Perform telephonic and face-to-face functional assessments for member needs.
• Coordinate care with health care team for chronic illnesses and disabilities.
• Develop and monitor member care plans with short and long-term goals.
• Interface with Medical Directors and Interdisciplinary Teams on care management.
