Responsibilities

• Perform telephonic and face-to-face functional assessments to identify and manage member needs.
• Manage members with chronic illnesses to ensure cost-effective and efficient care.
• Obtain thorough member histories to develop individualized care plans.
• Establish short and long-term goals with members and supports.
• Develop care plans and ensure access to services.
• Assist with authorizations/referrals within benefits or extra-contractual arrangements.
• Collaborate with Medical Directors and care teams on person-centered care plans.
