Job summary

Elevance Health is seeking a LTSS Service Coordinator – RN Clinician (RN Case Manager) in Ohio. This field-based role travels to client sites and may attend offices for meetings.

You will manage members’ care plans across physical, behavioral health, and long-term services, prioritizing person-centered care and coordination. Responsibilities include conducting assessments, coordinating with care teams, and developing individualized care plans.

Qualifications

• High school diploma or GED or equivalent.
• Current, valid RN license in Ohio.
• Minimum 3 years of experience with chronic illnesses, co-morbidities, or disabilities in coordination roles.

Responsibilities

• Perform telephonic and face‑to‑face functional assessments to identify and manage member needs.
• Identify high‑risk members and coordinate care with the health care team.
• Manage members with chronic illnesses to ensure cost‑effective use of benefits.
• Develop and monitor individual care plans with short and long‑term goals.
• Interface with physicians and interdisciplinary teams to adjust care plans as needed.
