Qualifications

• Reside in Illinois (Macoupin County & surrounding areas).
• Bachelor's or Master's degree in a human-services field.
• Two years of case management experience recommended.
• Reliable transportation and valid Illinois driver’s license.

Responsibilities

• Evaluate referred members’ needs/eligibility and plan care accordingly.
• Coordinate and monitor care plan activities and progress.
• Collaborate with clinical teams to improve care outcomes.
• Provide coaching and support to empower members in decision-making.
