Pair Team is expanding its field-based care management role to bring care directly to members in homes and community settings. The Field Care Coordinator will conduct in-person visits, onboard new patients, and provide hands-on support to overcome barriers to care.

The position requires 3–5 visits per week in the field, full-time hours, and regular travel within the assigned region, with a strong emphasis on relationship-building and coordination across care teams.

Qualifications

• 1+ years of professional experience in case management, care coordination, or community-based services.
• Ability to navigate healthcare and social service resources with cultural sensitivity.
• Strong written and verbal communication for diverse populations.

Responsabilités

• Serve as the primary contact for members with complex medical, behavioral, and social needs.
• Meet members in homes, communities, or partner sites to onboard and deliver care packages.
• Conduct in-person visits to assess needs and encourage adherence to care plans.
• Coordinate services and referrals across primary care, behavioral health, and social supports.
• Collaborate with Nurses and Behavioral Health Care Managers to ensure continuity of care.
• Identify barriers to care such as transportation or health literacy and address them.
• Develop individualized care plans and follow up on all interventions and goals.
• Document all member interactions accurately for care coordination.
• Advocate for member needs within healthcare and social systems.
• Participate in care team meetings to review patient panels and progress.
