Job summary

Pair Team in Orange County, CA is seeking a Field Care Coordinator to deliver care directly to members in homes and community settings. You will conduct in-person onboardings, deliver care packages, and collect health data during visits. Travel within the assigned region is required.

We require 1+ year of case management or care coordination experience, a valid driver’s license, and reliable transport. Proficiency with Google Suite, Slack, and CRM systems is expected.

Qualifications

• 1+ years of professional experience in case management or care coordination.
• Physical location in or near Orange County, CA with local knowledge.
• Valid driver’s license and reliable transportation for field visits.
• Strong cultural fluency and empathy when working with diverse populations.

Responsibilities

• Serve as the primary contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes, community sites, or partner locations for onboardings and visits.
• Conduct in-person visits to assess needs, support care plans, and encourage adherence.
• Coordinate services and referrals across primary care, behavioral health, and social supports.
• Collaborate with nurses and care managers to ensure care continuity.
• Identify barriers to care and help with transportation, access, and health literacy.
• Develop and maintain individualized care plans and ensure timely follow-ups.
• Document all interactions and care coordination activities accurately.
• Advocate for member needs within healthcare and social support systems.
• Participate in team care meetings to review panels and align on plans.
