Job summary

Pair Team is expanding field-based care management to improve engagement and access for members in California’s communities. The Field Care Coordinator will visit homes, community sites, and partner organizations, delivering care packages and collecting health readings as part of hands-on, relationship-based support.

This full-time role requires regular travel within your assigned region and in-person interactions three to five times per week to ensure continuity of care and adherence to care

Qualifications

• 1+ years of professional experience in case management, care coordination, or community-based services.
• Strong cultural fluency and ability to build trust with diverse populations.
• Reliable transportation for regular field visits and valid driver’s license with insurance.
• Excellent organizational skills and attention to detail.

Responsibilities

• Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes, in the community, or at partner organizations to onboard and deliver care packages or collect health readings.
• Conduct in-person visits to assess needs, provide ongoing support, and encourage adherence to care plans.
• Coordinate services, appointments, and referrals across primary care, behavioral health, and social support resources.
• Support care continuity by collaborating with Nurses and Behavioral Health Care Managers.
• Identify and address barriers to care such as transportation, medication access, or health literacy challenges.
• Develop and maintain individualized care plans and ensure follow-up on interventions and goals.
• Maintain accurate documentation of all member interactions and care coordination activities.
• Advocate for member needs within healthcare and social support systems.
• Participate in collaborative care meetings with the care team to review patient panels and plans.
