Job summary

Centers Plan for Healthy Living is seeking a Field Coordinator to support care management by scheduling and performing in-person visits at members' residences, coordinating with the Care Management team to meet each member's care needs. The role requires travel to member homes and adherence to regulatory guidelines while maintaining accurate documentation.

The ideal candidate has a High School Diploma, strong communication skills, and experience in customer service.

Qualifications

Responsibilities

• Schedule and conduct in-person visits at the member's residence.
• Review and complete visit documentation with the member or caregivers.
• Maintain up-to-date documentation for assigned cases.
• Follow care coordination documentation guidelines (problems, interventions, outcomes).
• Reach weekly caseload and provide daily report of completed visits.
• Coordinate resources and services with Care Management to ensure safe, effective care.
• Participate in team meetings and ongoing educational activities.
• Ensure compliance with CPHL policies, NY State, CMS, and Federal regulations.
• Other assigned duties.
