Job summary

UnitedHealthcare is seeking a Field Care Coordinator- HIDE SNP to coordinate care for LTSS recipients in a field-based role with a home office. Expect up to 75% travel within roughly 50 miles, with telecommuting flexibility near Berrien County, MI.

Responsibilities include care planning, stakeholder communication, and community resource referrals. Qualifications include Michigan RN licensure or LMSW/LCSW/LLMSW, 2+ years in community health, and proficiency with electronic documentation and MS

Qualifications

• Must have active licensure in Michigan (RN or Social Work).
• 2+ years in community health settings; LTSS/home-based care experience preferred.
• Familiarity with social determinants of health and community resources.

Responsibilities

• Develop and implement person-centered care plans across the continuum.
• Coordinate health information among stakeholders for timely care.
• Advocate for patients and families with the care team.
• Assess and plan care strategies customized to individuals.
• Identify barriers to care and initiate appropriate interventions.
• Refer to social services programs and community resources.
• Support member resources for local services and housing.
• Manage the person-centered service plan and conduct home visits.
• Gathers and maintains patient data per state/federal guidelines.
