Job summary

UnitedHealth Group is seeking a Field Care Coordinator to support Michigan residents in Berrien County and surrounding areas. You will serve as the single point of contact, develop individualized care plans, and coordinate with healthcare teams to ensure timely services.

The role is field-based with a home office, requiring up to 75% travel within a 50-mile radius and the possibility to telecommute from home in a suitable workspace. Sign-on bonus may apply for external applicants.

Qualifications

• Must have one of: RN, LMSW/LCSW/LLMSW, or LLBSW licensure.
• 2+ years in community health care setting.
• 1+ years working with SDoH and community support services.

Responsibilities

• Develop and implement person-centered care plans.
• Coordinate with families and health care team for timely services.
• Identify barriers to care and refer to social programs.
• Conduct home visits and coordinate with care team.
• Document care management activities per state/federal guidelines.
