Job summary

Pair Team is expanding its field-based care management role in Alameda County, bringing care to patients at home and in the community. The Field Care Coordinator will conduct in‑home visits, onboarding, and follow‑up, focusing on engagement and access to services.

You will work five days a week across communities, coordinating with Nurses and Behavioral Health Care Managers, and managing a high‑volume schedule during travel in the field.

Qualifications

• Bilingual English/Spanish.
• 1+ year field-based case management or care coordination.
• Physical location in or near Alameda County with familiarity of the local community and resources.
• Reliable transportation for regular field visits.
• Valid driver’s license and auto liability insurance.
• Strong cultural fluency and ability to build trust with diverse populations.
• High degree of empathy and ability to remain patient in adversity.
• Demonstrated ability to work collaboratively in a multidisciplinary care team.
• Excellent organizational skills and attention to detail.
• Strong technical skills and comfort learning new systems; Google Suite, Slack, CRM, Excel.

Responsibilities

• Manage a high-volume field-based caseload; ~30 in-person visits per week.
• Independently manage a busy daily schedule while traveling throughout Alameda County.
• Support recurring community-based events and care coordination for patients with complex needs.
• Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes, in the community or at partner organizations to onboard and deliver care packages.
• Maintain accurate and timely documentation of all member interactions and care coordination activities.
