Optum is looking for a Field Care Coordinator who will serve as the primary care manager for a panel of members with various health needs. This role requires extensive travel to meet members in homes or care facilities throughout Maricopa County, Arizona.

The ideal candidate has strong case management experience, particularly with serious mental illnesses, and is adept at developing person-centered care plans. They should also demonstrate excellent communication skills, adaptability, and a commitment to improving member outcomes through collaboration and advocacy.

Qualifications

• 2+ years of case management experience serving members with Serious Mental Illness.
• 1+ years of case management experience with elderly or disabled individuals.
• Access to reliable transportation to meet members.

Responsibilities

• Complete comprehensive needs assessments face-to-face or via phone.
• Develop and implement person-centered care plans.
• Partner with internal teams and community resources.
