Qualifications

• Bachelor's degree in Social Work or related field required.
• 3+ years of care coordination, behavioral health, or healthcare experience.
• 3+ years of experience with members having medical needs, elderly, or disabled.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Driver's license and ability to travel within territory.

Responsibilities

• Engage members to complete comprehensive needs assessments.
• Develop and implement person-centered care plans.
• Collaborate with internal care team, providers, and community resources.
• Provide education and coaching for member self-management.
• Support discharge planning and care transitions after ER/SNF events.
• Advocate for members and families with the health care team.
