Job summary

CVS Health Corporation seeks a Case Manager Registered Nurse – Field to deliver high-quality care coordination for members in Hudson County, NJ and surrounding areas. This full-time role includes field visits, home assessments, and collaboration with multidisciplinary teams to optimize health outcomes.

You will require an active NJ RN license, 3+ years of clinical practice, and the ability to travel up to 75% while working a 8am-5pm EST schedule.

Qualifications

• Active and Unrestricted RN license in NJ.
• 3+ years clinical practice experience in hospital or similar setting.
• Must possess reliable transportation and be willing to travel up to 75% in Hudson and surrounding counties, NJ.

Responsibilities

• Conduct face-to-face home visits for assessments using standardized tools.
• Develop and implement care plans with care team; coordinate services within MLTSS/ D-SNP/FIDE programs.
• Document accurately in the electronic health record.
• Coordinate with member, PCP, and care team to ensure safe discharge and appropriate services.
• Mentor new hires after achieving proficiency in the role.
